Please complete or update your student’s forms no later than August 18th.  If you have a NEW student or a student who will be issued a Bloom-Carroll email address for the first time, those students will be able to log in and sign their forms at school at the start of the new school year.   It is necessary for the forms to be updated and signed every year.

INSTRUCTION:

• Login to Final Forms (www.bloomcarroll.finalforms.com).

• If you do not have an account, create a parent account.

• Complete forms (new students) or update existing forms (current students) with any information that may have changed since last year.

• If you do not remember your login information, click on the “Forgot Password” link. You must use the same email address that you used to sign up initially.  If you do not know that email address, contact our IT department at 740-756-9798.
