Parents of incoming kindergarten students can begin scheduling screening appointments on April 6th. Parents/Guardians will register online through MyConferenceTime which can be found on our website under Bulldog Portal, Parent Resources, MyConferenceTime. This is the same online scheduling system that we use for Parent-Teacher Conferences.

Screenings will take place Tuesday, June 1st, Wednesday, June 2nd, and Thursday, June 3rd. Screenings will be held at the Bloom-Carroll Primary School, located at 69 S. Beaver St., Carroll, OH.  Children need to be 5 years of age by August 1st. The screening is for Bloom-Carroll residents only. We request that only one parent/guardian attend this screening appointment. This screening appointment can take up to an hour to complete. This includes a socially distanced meeting with a kindergarten teacher to discuss the results of the screening. *Due to COVID requirements, masks will be required to be worn at all times. 

Parents also need to complete Final Forms for the 2021-2022 school year prior to your screening appointment. The site will open on April 26th, and parents must choose the 2021-2022 school and select Kindergarten as the grade level. You can access Final Forms by visiting our website, clicking on Bulldog Portal, Parent Resources, Final Forms. Failure to complete and sign all forms prior to your screening appointment could result in a screening delay or rescheduling of appointment.

If you are a new parent to the district, click on New Account and follow the instructions to create your account and add your students. Forms are required to be signed each school year, and any time there is a change to your child's account, including phone number, emergency contacts, address, etc. Please keep your username and password in a secure place so that you can access Final Forms in the future. If you have a change of address, you must notify the district registrar and provide proof of new residence.
