Dear Parent/Guardian,
COVID-19 is a very contagious respiratory illness caused by the SARS-CoV-2 virus which was first identified in 2019 in Wuhan china. In accordance with section 3707.09 of the Ohio Revised Code, if infected with COVID-19, you may pose a threat to the health of the public and others close to you. Your student, _____________________, is being sent home with COVID-19 like symptoms on this date: _________ and will not be able to return without an excuse from a Primary Care Physician (PCP) or designee (Physician Assistant or Nurse Practitioner).
Your student may return to school if they complete a symptom-based strategy if you choose not to have your student see a physician or designee. This strategy would start on this date: _____________
and they would need to meet all the criteria below:
Symptoms include but not limited to:
fever
cough
sore throat
runny or stuffy nose
loss of taste or smell
nausea and vomiting
If we are only sending your child home with a fever, and no other symptoms, they may return after 24 hours of being fever free without the use of fever reducing medications.
At any time if you or your student have trouble breathing or other emergency complications call 911 or immediately go to the emergency room to be evaluated.
Your student will need to quarantine until they have seen a physician or designee and have an excuse from them, or they have met all the criteria of the symptom-based strategy if they are sent home with this letter. The Fairfield Department of Health will be notified of this letter.
