The Bloom-Carroll Primary Kindergarten Screening will take place Wednesday, May 30th, Thursday, May 31st, and Friday, June 1st. The screening is for Bloom-Carroll residents only and will be held at Bloom-Carroll Primary School located at 69 South Beaver Street, Carroll, Ohio.
Pre-registration opens on Monday, April 9th. You may call the school office anytime after this date to begin scheduling an appointment. Please be aware that children need to be 5 years of age by August 1st.
At the time of screening, you will need to bring the following documents:
- Birth Certificate
- Proof of Residence (One of the following: mortgage papers, lease agreement with landlords name and phone number, car registration, bank statement, tax papers, electric or utility bill)
- Immunization Records
- Parent Picture ID
- Custody Papers (if applicable)
- If a foster child we need legal court documents
- Special Education Records (IEP, ETR) if applicable
