7th and 8th Grade Immunization Letter

Dear Parents or Guardian,

In Ohio, certain vaccines are required by law for attendance in 7th grade and 12th grade.  To fully protect your teen, they will need all of the recommended vaccines because (1) some immunity fades over time and (2) teens are at higher risk for some diseases during their teen and early adult years.
